Have the kids fill out the questionnaire and draw a little picture of their dad. If the kids aren’t old enough to write, ask the questions and write down the answers for them.
There is also a place for kids to write their name, age, and date. The honest answers will probably give the dad a good laugh!

- Cardstock (you can also use office paper).
- Pen or pencil.
